Patient Care Manager - Home Health | RN Leadership Role in Philadelphia, PA

Key Responsibilities

Lead and oversee patient care operations ensuring delivery of high-quality home health services
Ensure full regulatory compliance with Medicare Conditions of Participation, CMS guidelines, and all federal, state, and local healthcare requirements
Supervise, mentor, and develop nursing staff to maintain clinical excellence and professional growth
Collaborate with interdisciplinary teams to develop and implement personalized, patient-centered care plans
Monitor and ensure timely, accurate clinical documentation and electronic medical record (EMR) management
Conduct regular audits of patient records for quality assurance and compliance verification
Facilitate effective communication among patients, families, physicians, and healthcare team members for seamless care coordination
Lead quality improvement initiatives and performance improvement projects
Promote continuous professional development and evidence-based practice standards
Manage staffing schedules, patient assignments, and resource allocation

Required Qualifications

Active Registered Nurse (RN) license in good standing in the state of Pennsylvania
Minimum 2 years of management or supervisory experience in healthcare setting
Previous experience in home health care or home care services , strongly preferred
Comprehensive knowledge of accepted professional nursing standards and clinical best practices
Thorough understanding of Medicare Conditions of Participation (CoPs) and OASIS documentation
Strong familiarity with federal, state, and local healthcare regulatory requirements
Excellent leadership, communication, and interpersonal skills
Proven ability to mentor clinical staff and build high-performing teams
Strong critical thinking and problem-solving capabilities
Proficiency with electronic health records (EHR) and healthcare management systems
Bachelor of Science in Nursing (BSN) preferred
