Patient Care Manager - Home Health | RN Leadership Role in Philadelphia, PA

Key Responsibilities

• Supervise, mentor, and develop nursing staff to maintain clinical excellence and professional growth
• Collaborate with interdisciplinary teams to develop and implement personalized, patient-centered care plans
• Monitor and ensure timely, accurate clinical documentation and electronic medical record (EMR) management
• Conduct regular audits of patient records for quality assurance and compliance verification
• Facilitate effective communication among patients, families, physicians, and healthcare team members for seamless care coordination
• Lead quality improvement initiatives and performance improvement projects
• Promote continuous professional development and evidence-based practice standards
• Manage staffing schedules, patient assignments, and resource allocation

Required Qualifications

• Active Registered Nurse (RN) license in good standing in the state of Pennsylvania
• Minimum 2 years of management or supervisory experience in healthcare setting
• Previous experience in home health care or home care services , strongly preferred
• Comprehensive knowledge of accepted professional nursing standards and clinical best practices
• Thorough understanding of Medicare Conditions of Participation (CoPs) and OASIS documentation
• Strong familiarity with federal, state, and local healthcare regulatory requirements
• Excellent leadership, communication, and interpersonal skills
• Proven ability to mentor clinical staff and build high-performing teams
• Strong critical thinking and problem-solving capabilities
• Proficiency with electronic health records (EHR) and healthcare management systems
• Bachelor of Science in Nursing (BSN) preferred
