Licensed Vocational Nurse (LVN) – Weekend Shift - Hospice
Location: Ontario, California (On-site)
Work Type: Full-Time
Shift Schedule: Saturday and Sunday + 1 weekday, 8:00am - 8:00pm
Compensation: $28 – $32 per hour, depending on experience

Position Summary
The Licensed Vocational Nurse (LVN) is responsible for providing direct patient care in a hospice setting under the supervision of a registered nurse. The LVN collaborates with the interdisciplinary care team, implements care plans, delivers prescribed treatments, and supports both patients and their families throughout the care journey.
Responsibilities
Deliver direct nursing care in accordance with the State Nurse Practice Act
Follow the plan of care established by the RN or clinician
Provide timely and accurate documentation consistent with care standards
Educate patients and caregivers regarding diagnosis, treatment, and care expectations
Coordinate with other disciplines such as PT, OT, MSW, HHA, ST, and dietitians
Utilize equipment and supplies efficiently and effectively
Participate in ongoing professional development and training
Complete other duties as assigned
Participate in monthly RN supervision sessions as required by agency policy
Qualifications
Graduate of an accredited vocational or practical nursing program
Active LVN/LPN license in the state of California
Minimum of three years of nursing experience preferred; hospice, home health, or med-surg experience ideal
Valid driver’s license and insured, reliable vehicle required for travel
Current BLS Hands-On CPR Certification
Blood draw and/or IV certification preferred
Strong communication, documentation, and organizational skills
Ability to work independently while maintaining a high standard of patient care
