Job Description
Are you passionate about patient care and helping individuals manage their health? We're looking for a dedicated Care Coordinator to provide remote support to patients managing hypertension and other chronic conditions. This is an independent contractor (1099) position, offering a flexible schedule while making a meaningful impact in healthcare.
What You'll Do:
Provide ongoing support to patients by monitoring their blood pressure and making follow-up calls.
Assist patients with understanding and managing their hypertension by reviewing daily readings and offering education.
Support the care plan established by healthcare providers to ensure optimal patient outcomes.
Verify and document patient information, including demographics, medical history, medications, and risk factors, using specialized medical software.
Reach out to patients when their readings exceed normal parameters and assess their needs.
Collaborate with healthcare practices and providers when medical attention is required.
Help patients access timely medical support when urgent concerns arise.
Register patients in Chronic Care Management (CCM) and Principal Care Management (PCM) programs and ensure proper documentation.
Monitor adherence to care plans, assess effectiveness, and adjust plans as needed.
Provide patient counseling, education, and instruction.
Maintain compliance with HIPAA guidelines to protect patient confidentiality.
Handle sensitive patient conversations with professionalism and care.
Participate in individual and group projects as needed.
Conduct audits of Chronic Kidney Disease (CKD) patients to ensure they have scheduled appointments.
Perform other related duties as assigned.
What You'll Need:
✅ Licenses & Certifications:
LPN or CCMA Certification (Required)
✅ Skills & Qualifications:
Strong knowledge of chronic disease management, including chronic kidney disease, diabetes, COPD, and congestive heart failure.
Excellent time management, communication, and organizational skills.
Ability to provide exceptional customer service and demonstrate a patient-centered approach.
Comfortable with Windows-based computer systems and proficient in Microsoft Office (Word, Excel, Outlook).
Previous experience working with an electronic health record (EHR) system is a plus.
Ability to identify problems and recommend effective solutions.
✅ Experience:
At least 1-3 years of experience in a healthcare setting, preferably working with patients managing chronic conditions.
Prior experience making patient calls, particularly with U.S.-based clients, is preferred.
Work Requirements:
Fully remote position - must have a secure home office that complies with HIPAA regulations.
Must provide your own workstation (Windows desktop/laptop) and headset.
Internet speed requirements: Minimum 100 Mbps download / 20 Mbps upload.
Must be comfortable speaking on the phone throughout the day.
Ability to communicate fluently in English; additional languages are a plus.
