Job Description

Make a Real Impact as a Community Violence Intervention (CVI) Care Manager! Join ACE’s frontline team to empower individuals and families affected by gang violence, providing life-changing case management through the Family Case Management (FCM) Model and Transitional Client Services . Collaborate with Community Intervention Workers (CIWs) to deliver comprehensive support, from outreach and intake to individual and group services. Your work will help break cycles of violence, create safer communities, and offer hope for a brighter future. 

ACE’s CVI program serves ages 16 to adult, who are gang-affected or otherwise at risk of gang involvement. Through outreach activities and incident response, the program promotes community-level peacemaking and violence reduction. Through a phased program centered around family case management services, the program works to strengthen family ties, goal attainment, and other pro-social influences.

DUTIES AND RESPONSIBILITIES: 

  • Performs outreach and marketing duties to recruit gang-impacted participants and educate the community on ACE services.
  • Develops individualized care plans, focusing on coordinating healthcare, housing, and social services for high-risk participants in compliance with funders/ACE/ECM standards.
  • Advocates on behalf of participants with healthcare providers to ensure proper care and coordinates transitions between healthcare settings and social service providers.
  • Leads/participates in intensive case management and care coordination for participants with complex health and social needs, including connecting them to medical, behavioral health, and community-based services.
  • Leads/participates in family and service team meetings and coordinates wrap-around ECM services with other providers.
  • Partakes in mediation and advocacy as necessary, conducting home visits in high-crime areas, hostile environments, courts, and with probation officers.
  • Provides safe passage across gang lines to gang-impacted participants to ensure their safety in accessing necessary services.
  • Organizes, coordinates, prepares, and delivers skills-based workshops for gang-impacted participants and their families.
  • Organizes, coordinates, and attends intentional field trip activities with gang-impacted participants and their families.
  • Provides case management for an average standing caseload of 20 participants, serving about 25–30 Family Case Management participants annually.
  • Responsible for administering the SET assessment tool.
  • Assists participants in obtaining Work Ready documents and/or making referrals to partner organizations.
  • Organizes celebration activity meetings with gang-impacted participants and their support systems.
  • Collaborates with MCO case managers to align care coordination efforts and share participant progress.
  • Supports MCO ECM metrics through regular reporting on health and social outcomes.
  • Carries out intake and eligibility determinations and makes appropriate referrals when applicants are not eligible for ACE services.
  • Completes and updates participant service plans based on ongoing assessments.
  • Documents all interactions and ensures compliance with funders/ACE/HIPAA regulations.
  • Enters data into multiple participant databases; assists in writing periodic program reports.

KNOWLEDGE, SKILLS AND ABILITIES: 

  • Possesses understanding of Enhanced Care Management principles, including frequent participant contact, care plan development, and collaboration with Medi-Cal Managed Care Organizations (MCOs) and community-based organizations.
  • Understands complex care management principles and can develop care plans for participants with chronic health conditions and social needs; demonstrates strong care management skills, including monitoring care plan adherence and coordinating multidisciplinary services.
  • Understanding of Medi-Cal systems and the social determinants of health affecting underserved populations, with a willingness to build expertise in these areas.
  • Knowledge of community/gang dynamics within the GRYD Zone.
  • Able to operate within ACE’s designated GRYD zone, exposing oneself to gangs found within the zone.
  • Able to work in assigned locations that may expose oneself to hazardous conditions or in extreme physical discomfort or distress.
  • Ability to assess conditions and responsiveness to interventions.
  • Possesses necessary de-escalation skills to provide crisis and community violence intervention in the community.
  • Possesses knowledge of programs, regulations, and procedures regarding human services, social services, justice, and workforce development.
  • Is familiar with standard approaches to accurate participant record keeping, including timely charting, notation formats, content limitations, and confidentiality practices.
  • Recognizes appropriate boundaries and limitations on interactions with program participants and their families, maintaining professional standards and practices.
  • Is sensitive to community, culture, and personal dignity of program participants and is able to successfully engage, assess, and plan services for participants typical of the program’s target population.
  • General proficiency in navigating and using technology.

PHYSICAL REQUIREMENTS: 

  • Prolonged periods standing and walking throughout facilities.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
  • Must be able to bend, lift, stretch, climb, and crawl to provide services.


QUALIFICATIONS: 

  • Bachelor’s Degree in relevant field and 2+ years of gang intervention experience; completed Community Violence Intervention training, or
  • Equivalent combination of training and experience
  • Spanish language skills, verbal and written preferred  
  • Possess a current California Driver’s License, clean DMV record & adequate auto insurance. 
  • Must submit to a Department of Justice Criminal background check 
  • Initial TB employment clearance required

The anticipated pay range for candidates who will work in California is $28.85 to $30.45 hourly. The offered pay to a successful candidate will be dependent on several factors that may include but are not limited to years of experience within the job, years of experience within the required industry, education, etc.
