We are seeking a compassionate and dedicated Care Manager to join a supportive team that partners with individuals with intellectual and developmental disabilities (I/DD). In this role, you will help coordinate and deliver the necessary services to assist individuals in achieving their health, wellness, and life goals. Key Responsibilities:
Conduct comprehensive assessments to understand the clinical, psychosocial, and service needs of individuals.
Develop, implement, and update person-centered Life Plans that incorporate both clinical and non-clinical healthcare needs.
Provide health promotion and support to help individuals maintain optimal health and wellbeing.
Coordinate chronic disease management and access to necessary care.
Facilitate referrals to clinical and community resources, including planning and follow-up for transitional care.
Collaborate with an interdisciplinary team to ensure all needs are addressed in a holistic and integrated manner.
Engage with families and natural supports to foster a collaborative care process.
Advocate for individuals, promoting self-advocacy and independence.
Ensure services are culturally and linguistically appropriate.
Use Health Information Technology (HIT) for documentation and service coordination.
Maintain confidentiality and adhere to HIPAA regulations.
Knowledge, Skills, and Abilities:
Strong crisis management skills and the ability to assess and act quickly.
Basic technology skills, including the use of health records and documentation.
Excellent time management and organizational skills.
Professional work habits, including dependability and responsibility.
Ability to maintain boundaries and ethical standards in a professional setting.
Education and Experience Requirements:
Bachelor's degree with two years of relevant experience OR
A license as a Registered Nurse (RN) with two years of relevant experience (including but not limited to case management/service coordination) OR
A Master's degree with one year of relevant experience.
